Vaccine Information
PA Vaccination Requirements
Per Pennsylvania law, vaccinations are required for children to attend school. Student immunizations should be current for their grade as of the first day of school. Please know that students may be excluded from school if they do not meet the vaccination requirements for their grade or if there is an outbreak of a vaccine-preventable disease and your child is exempt from vaccination. Written proof of immunizations is required, which can be one of the following:
- Medical baby book that your child's doctor completed at the time of immunizations.
- A written list of immunizations and dates which the doctor’s office has provided on his/her letterhead.
- A physical examination form with the dates of immunizations listed.

VACCINATION DOSES Diptheria, Tetanus & Pertussis 4 doses (one dose must be on or after the 4th birthday) Polio 4 doses (4th dose on or after the 4th birthday and at least 6 months after the previous given dose)
Measles, Mumps & Rubella 2 doses
Hepatitis B 3 doses
Varicella 2 doses of varicella vaccine or evidence of immunity

VACCINATION DOSES Diptheria, Tetanus & Pertussis 1 dose Meningococcal 1 dose
